Tongue-tied is a diagnosis given only if routine functions such as eating, speech, and sleep are being impaired. The lingual frenulum is usually either too tight or too forward. We don't give diagnosis based on appearance. The diagnosis is reached by three main reasons, mother signs & symptoms, baby signs & symptoms, and objective findings on baby (white patch on the tongue, high-arched palate, lip blister, cobblestone appearance to lips, redundant gum pads or overgrown baby gum pads.
The lingual frenulum doesn't have to touch the tip of the tongue to cause the pain, sometimes the frenulum is visible with retraction by an experienced clinician.
